Individuals diagnosed with type 2 diabetes (T2DM) must undertake self-management skills to manage the disease and avoid complications. United States (U.S.) military veterans have a higher incidence of T2DM than general U.S. population. The increasing medical burden caused by the care of those with diabetes requires innovative care strategies. A quantitative, quasi-experimental design was used to evaluate the impact of the SMA on HgbA1c values and rates of patient satisfaction. Participants were assigned to primary care VA community-based outpatient clinic in Arizona. Dorothea Orem's self-care deficit nursing theory and Bandura's social cognitive theory were used to form the project's theoretical base. A total of 16 patients in the PCP's panel of patients met inclusion criteria at this time of having HgbA1c levels greater than 9.0%. Eight patients qualified for inclusion and agreed to participate in the project. Five participants completed the SMA series. Paired t-tests were used to compare pre-intervention and post intervention HgbA1c levels and patient satisfaction scores. The PSQ-18 survey was used to score satisfaction in seven categories. A significant statistical difference (1.14%) was noted in comparing pre and post- HgbA1c levels (p = .002) for participants. There were not statistically significant differences in the pre-intervention and post intervention patient satisfaction scores as noted by a difference of -0.27 (p = 0.0593). The SMA was implemented with ease in the primary care setting. Participants achieved statistically significant reduction in HgbA1c levels. Further research is needed using larger sample sizes to improve the strength of the findings.
